一、假條的上方應該按照正式的格式寫(xiě)上如下信息：
To：假條是遞給誰(shuí)的To：Peter Stone，
From：請假人From：Lynn Chen，Financial Department Date：寫(xiě)假條的日期（注意不是請假的日期）Date：April 2nd，20xx
Subject：寫(xiě)上請假字樣Subject：Casual Leave of Absence
二、在您的請假信第一段，應該開(kāi)門(mén)見(jiàn)山但是有禮貌地提出請假.第一段要中心明確，寫(xiě)清您要請假的日期.
